Acceptable forms of proof are as follows.

Veteran

As a veteran, or part-time National Guard or Reservist, you satisfy these conditions with your DD Form 214 showing the dates of your military service, or a current military ID card if still on active duty or a member of the National Guard and Reserves.

Service Dog

Provide documentation about the needs your service dog fulfills and the contact information for the trainer or training organization.

Emotional Support Animal (ESA)

Proof of an emotional support animal (ESA) is an official letter from a licensed mental health professional. The letter must confirm that the owner has a qualifying mental health condition and that the pet helps them deal with it. The letter should include the owner's name, date of birth, and the name and certificate or license number of the pet.
